This study confirms the clinical impressions of the majority of spine surgeons. Patients with degenerative spondylolisthesis fare better with surgery compared to non-operative management. If this study results in an increase in the utilization of fusion surgery to treat spondylolisthesis, the device manufacturing companies, such as Medtronic, DePuy, Nuvasive, and others, may see increases in their sales. CMS may see a significant increase in the number of fusion surgeries performed as a result of this study. CMS may see less variability in the rates of fusion surgery around the country as a result of this study.

Medtronic is likely to be able to get major commercial insurance carriers to cover reimbursement for cervical artificial discs The current failure of lumbar disc replacement should not be extrapolated to draw parallels and make conclusions regarding cervical disc replacement. It is naive to think that cervical disc replacement will replace cervical diskectomy and fusion operations altogether, but there are clinical scenarios where cervical disc replacement might be a better option for the patient.

While there are intuitive advantages to disc replacement therapy, the reality is that Medtronics trial design only showed equivalency rather than superiority to traditional fusion and considering the fact that most insurance companies do not pay for disc replacements and the huge body of experience with fusion may make adoption of this technology slower than anticipated. Once more data is collected to convince insurers and patients of the superiority of this technology, disc replacement may become the treatment of choice for degenerative disc disease.

Most cartilage repair techniques must take into account the age and biology of the area being treated. We all know that the aging process significantly decreases the healing potential of most collagen based procedures, whether that be skin, tendon, ligament, cartilage or bone. The myriad of techniques including microfracture Autologous OATS, allograft OATS, ACI, syntheic fillers and resurfacing all have their ideal pattients and in most articles, the confounding variables are poorly controlled for. Resultrs will always be better in the healthy, isolated lesion in the non-smokers, younger patient without alignmenet issues. In order to get a cohort of ideal patients, the numbers to independently assess all these technologies would be staggering. The best ideas at this point seem to be the initiaion of the most conservative treatment with progression to more invasive techniques upon determining failure of results with previous procedures.
