The concept of re-filling the empty lens capsule following lens or cataract extraction with an elastic substance that mimics the accommodative function of the youthful human lens remains a captivating concept for researchers. Nishi's review article highlights the progress so far in this direction, as well as the challenges inherent in development of a successful design. These challenges include sealing the capsule to prevent leakage of the injectable material, adjusting the refractive state for emmetropia, allowing adequate accommodative amplitude and preventing capsular opacification.

Although a perfect vision is pursued by every ophthalmic surgeon, it is very important to be sufficiently modest to admit that, in most cases, that goal os not achievable. Cornea incison, posterior capsule debris or opacification, macular degeneration, error on intraocular lens power calculation, here are some of the potencial causes of insatisfaction if perfect vision is offered to patients before surgery. Lens refilling is an intelligent and original approach to accomodation loss after cataract surgery but there are still a lot of answered questions that will make patient selection a crucial issue when considering the use of this kind of lenses .

As baby boomers now make up a substantial part of many eye care practices, several expanding technologies will address their visual needs. Though current options to correct for presbyopia may have certain limitations with visual acuity, this option truly looks at restoring accommodation. It has a lot of promise to our aging patients.

My viewpoint of the staged acquisition of Alcon by Novartis focuses on the implication to Alcon R&D. At least two key shifts will occur (and in fact have already started)... 1. R&D activities will become much more collaborative. In the past, Alcon has developed many of their key technologies in house (witness the acrylic monomer for Acrysof). Now, I would expect to see key developments done in collaborations with universities and other technology based companies. This may result in lower development costs for Alcon, but will likely give competitors and the public an earlier view into their pipeline. 2. R&D senior management will include those with medical backgrounds. This has already begun (note Alcon's recent appointment of Sabri Markabi, MD from Novartis as SVP R&D). This will of course ensure a tight bond between management and clinician/surgeon, but may challenge the new management to understand the complexities inherient in the introduction of new technologies.
