I have a patient with non small cell carcinoma of the lung who has had a great response for over seven years on this medication. She was a perfect fit- female, non smoker. After seven years she is NED. She was close to being referred to hospice seven years ago. She is receiving the med at present free of charge from the company. Metastatic spread to the bone is a common and problematic complication for many patients with advanced cancer.  Current treatment with bisphosphonates only modestly controls skeletal spread. This study suggests that the monoclonal antibody Denosumab has biologic and clinical activity to help patients who are at risk for further bone metastases despite bisphosphonate therapy.

Small Cell lung cancer is tough to treat, especially when it recurs after prior therapy, and treatment options are limited.  Picoplatin is a new-generation platinum chemotherapy, and is being studied by Poniard Pharmaceuticals (PARD) in a randomized, 400-patient, phase III study.

This is an excellent paper to demonstrate the importance of a good phase I trial with detailed pharmacokinetics, dosing and continuous vs. bolus administration of each anti cancer drugs. The finding regarding low dosing of this drug as causative of enhancement of cancer should not effect the potential use of this drug, especially in combination with anti VEGF-R2 drugs currently in use.

Swedish company Santaris has developed several RNA antagonists to key protein targets in the biology of cancers. Two of those are in collaboration with ENZON for solid tumors. One targets hypoxia-inducible factor 1 alpha (HIF1a), the second targets survivin, a protein shown to protect rapidly growing cells (cancer and fetal) from apoptosis and is also implicated in the cell cycle. Technology risk is being managed with advance of multiple targets into early trials. The challenge will be in making sound choices before Phase III.

Catumaxomab is a trifunctional monoclonal antibody with 2 antigen-recognition sites: one for human EpCAM, a cell surface antigen expressed by a variety of epithelial tumor cells, and one for human CD3, a T-cell surface antigen. In addition, another region of the antibody binds to receptors that are expressed on other cells of the immune system.

Cautious optimism appropriate for a cancer with a historically dismal outlook.

Many adenocarcinomas and sqamous cell cancers express HER2 . However, HER2 based therapy is only approved therapy in early and late stage breast cancer. 25% of gastric cancers express HER2. The Toga trial was conducted practically everywhere except the US; indeed there are geographic differences in gastric cancer in regards to risks, location, age of diagnosis, stage at presention, etc.. which are notable. However, the trial appears well powered between arms. The ToGA trial used a somewhat outdated regimen as control (5FU +CDDDP) however for study purposes that was appropriate. Other regimens will be compared to the H+chemo arm across studies (TPF, regimens investigated in REAL, etc..). I would assume minimal toxicity was added with H (aside from cardiac). Although we should all be optimistic (gastric cancer has a 5 yr median survival of 10-15%; incidence of proximal gastric and GE cancer is increasing more than any other cancer) results will need to be scrutinized when available.
