-Wyeth has submitted its new and improved pneumococcal vaccine for European approval. Although the current product (Wyeth's Prevnar) works well, the newer version would cover more serotypes of strep pneumo. -Since the original product is Wyeth's, they've been down this road before and know what the issues are likely to be to get approval; odds are very good that the new product will get approved on the first go-round or soon after

With the introduction of Prevnar, there has been a dramatic decline in invasive pneumococcaal disease in young children that has had a herd immunity effect on all ages. There has been a bit of a compensatory increase in serotype 19A disease (not in the current formulation of Prevnar). This new vaccine will add another six serotypes of protection, including 19A that will give us another weapon against the villain microbes.

This vaccine marks an improvement on the currently available pneumococcal vaccine for infants and children, which covers seven pneumococcal serotypes. Some of the additional serotypes have been increasing in prevalence since the seven types in the current vaccine have decreased, and it is logical to continue to aim at preventing new threats.

Vaccine manufacturers should rush for a seat at the table as this issue gets discussed. While the formulation of Tamiflu makes it easy to take, pandemic vaccines now in development should be considered for similar treatment.

Most American families are not familiar with signs and symptoms of influenza. Asking them to use (and not use) a home supply of antivirals would be insanity, stretching thin the amount of antivirals available and depriving them to those in true need. To ask them to use these meds appropriately for their children would be even more insane - at the first viral URI of the season the meds would be gone and parents would be on the phone for more, convinced that they had successfully fended off avian flu and eager to do it again.

The key point in this article is that the author shows a connection between Community Acquired clostridium difficile infections & lack of antimicrobial use. For example, the widespread use of ant-acids ie; proton pump inhibitors. This article is helpful because delineating some of the risk factors may help curb this deadly disease. However, make no mistake there is a very clear link between hospital or nosocomial transmission & antibiotic use. All patients with diarrhea requiring hospital admission (especially with a profoundly elevated white blood cell count) should be tested for C. difficile.
