Novexel's NXL104/ceftazidime Combination Commences Second Phase II Clinical Trial in Hospital Patients With Complicated Intra-Abdominal Infections (cIAIs) | www.pipelinereview.com

Novexel's trial of their novel beta-lactamase NXL104 combined with ceftazidime and metronidazole vs meropenem is an interesting choice.  While the Novexel arm looks to have very good activity against gram negative organisms and anaerobes, it almost completely lacks coverage against gram positive agents.  This appears to be a trial set to fail.

New technology has been developed that may play a role in the treatment of sepsis and hypoostension. This involves the development of a vagal nerve stimulator. A start-up company based in New Jersey- Electocore, has looked at animal models of epsis and hypotension and has found significant merit in their unique electrical signal for nerve stimulation to treat hypotension related to sepsis.

TAK-242 is a novel, selective inhibitor of Toll-like receptor 4 (TLR4) signaling. TLR4 is the receptor for the "endotoxin" LPS, which is released from pathogens and causes sepsis and multiple organ failure. In contrast to many previous failed antisepsis drugs, TAK-242 appears to be a common pathway inhibitor, and as such has a better chance of success if the right patient population is treated.  The Phase III trial has been stopped after an interim analysis by the DSMB found not sufficient cytokine lowering by the drug.In my opinion, the patients, who would benefit most are the ones with "moderate"  or early stages of sepsis ( in advanced stages, no drug works!) and should be screened for ther specific microbial infection which would respond to the drug best.

Good judgement and prudence on the part of the medical profession is the key to using antimicrobials to treat infectious diseases.  Education, correct diagnosis, proper use of laboratory medicine, bacterial isolation, antimicrobial susceptibility all need to accompany the use of antimicrobials.
