MADIT CRT was designed to assess if an ICD with CRT (vs. ICD alone) affected a combined endpoint of death or heart failure events in patients with mild cardiac symptoms. Over a mean of 2.4 years, the endpoint differed between groups due to a 41% reduction in heart failure events. Those who benefitted the most were those with a QRS of at least 150 ms. Mortality was similar with or without CRT; it was low. ICD with CRT decreased risk of heart failure events in this population.

Acute coronary patients who undergo angioplasty & stenting are treated with 2 inhibitors of platelet aggregation: aspirin, & clopidogrel. Recently we've seen a new entry to the market that has been nearly monopolized by clopidogrel, in Lilly's drug prasugrel. And now a 3rd drug, ticagrelor, looks likely to follow. There are significant but subtle differences among the 3, and clopidogrel will have a generic version soon, presumably further increasing the cost advantage for the older drug.

The MADIT CRT trial evaluated the benefit of cardiac resynchronization therapy for patients with reduced ejection fraction and mild cardiac symptoms. Previous studies have shown a benefit of CRT for patients with more severe cardiac symptoms. The trial demonstrated a significant reduction in the composite endpoint of heart failure events and death from CRT therapy. The of benefit was in the heart failure events component of the combined endpoint (as opposed to the mortality component).

In patients with atrial fibrillation, rates of stroke and embolism are similar comparing dabigatran, 110 mg, to adjusted dose warfarin. Rates of major hemorrhage were lower with dibigatran. Dibigatran at a 150 mg fixed dose is associated with lower rates of stroke and systemic embolism but similar rates of major hemorrhage compared to warfarin. It has taken a long, long time form oral renin inhibition to come along, and Novartis is seeking to target glomeruli on 5 continents. The South East Asian market is potentially huge for the treatment of hypertension and Japan has a long tradition of willingness to participate in the battle to lower blood pressure by all means pharmacological. Whilst there are still few data to demonstrate superiority of Direct Renin inhibition over ACE inhibition in long term outcomes, the field of hypertension treatment is still expanding, no doubt in part, driven by consumer demand for the newest agent, and the side-effects, real, important or otherwise of current agents often reported more in anecdote than evidence based format. Novartis has an important role in the area of Renin-Angiotensin System inhibition, and this new approval will do no harm to that reputation and market share.

The effect on reduces LDL cholesterol is more important so that reduce the risk of atherosclerosis. The effect on C-reactive protein is maybe one of the explication. Maybe other effect unknown. The adverse effect are not important. The impact must be important. We must prescribe more atorvastatine and less simvastatine.
