Implications: While no more would argue that control of systolic hypertension is crucial to preventing the consequences of hypertension such as congestive heart failure, MI's, left ventricular hyperthrophy, stroke and kidney failure there are other factors that are associated with these disease states. One of these is proteinuria.

Analysis: With the importance that proteinuria has in the development of cardiovascular complications, it makes excellent sense to use Tekturna and hydrochlorthiazide together as an initial starting therapy to control hypertension. As most patients need 3-4 different medicines to keep there blood pressure under 140 systolic, when additional drugs are needed the ones selected to be added to therapy should be angiotension receptor blockers and angiotension converting enzymes inhibitors, all of which, like Tekturna, lower both the blood pressure and proteinuria of patients being treated with them.

Drugs like Tekturna that attack the Renin Angiotensin System (RAS) not only reduce blood pressure, but just as importantly,  reduce proteinuria. Tekturna has been shown to reduce proteinuria 20% in a study that compared it to losartan in the control group. Both groups were treated with losartan while the active arm had Tekturna added to the other drugs patients were taking.

There are numerous studies that show that patients that have a higher 24 hour proteinuria rate have a higher incidence of cardiovascular and renal complications. Microalbuminuria (proteinuria < 300 mg/24 hours) is a risk factor for ischemic heart disease.

I believe that drugs that control blood pressure at the same time they reduce proteinuria, such as Tekturna, are the best agents to use in hypertensive patients, both diabetic and non-diabetic.
