Current replacement methods for extracted crystalline lens include accommodative intraocular lens that are enthusiastically being incorporated in current practice.  Refilling the capsular bag is an exciting concept but no successful technique is currently available.

Although the lask procedure as the primary method of keratorefractive surery has been under scrutiny recently it remains the mainstay of refractive surgery.  With a 95% approval it is difficult to critisize its effectiveness. However the 5% patient complaints must be addressed.  The risk of creating an unstable cornea by excessive thinning is minimized by careful pre-operative selection.  Surface ablation (PRK) is becoming more utilized and for higher powers phakic IOLs are becoming a popular method.  In cases of presbyopic hyperopia clear lens extraction and the implantation of an accommodating IOL is favored. Refractive surgery is entering a new phase where corneal surgery and intraocular lens procedures will be significantly incorporated into refractive surgery  management.

Glaucoma is an elusive diagnostic dilemma.  The aim of treatment is to lower the intraocular pressure below 18 mm of mercury.
However in the practice of ophthalmology a single reading on the quarterly exam fails to demonstrate the fluctuations of pressure that
may cause optic nerve damage in the interim.  The diurnal fluctuations from morn till eve may elude detection unless a morning and evening test is made.  Therefore true control of
pressure and avoidance of nerve damage is never completely
accomplished because of unknown pressures during the prolonged period of non-examination.  The contact lens constant monitoring of
intraocular pressure would be a major contribution to the understanding and more efficient treatment of glaucoma.  Every
ophthalmologist would be remiss if he failed to monitor all his
glaucoma patients to verify proper management.  This patent will
have a dramatic effect on the practice of ophthalmology and I'm
sure this device will occupy the examination table of a vast majority of eye care professionals.

While Benzalkonium Chloride (BAK) is the most common eye drop preservative (85% of all drops) it has an adverse effect on the ocular surface.  With prolonged use i.e. dry eyes and glaucoma preservative toxicity because a serious problem.  Currently uni-dose artificial tears are available but are objectionable as to cost and convenience but there are no preservative-free glaucoma drops.

Recently a company (Naturaceutical Delivery Corporation and Cis-Pharma) announced a multi-dose preservative free system that uses a one way valve technology that prevents air and microorganisms from entering  the product.  This allows shelf life to equal use life.  This technology would prevent package contamination and avoid the complication that currently faces the AMO contact lens solution.

The recent development of fungus infection causing serious vision threatening corneal infections worldwide has prompted Bausch and Lomb to withdraw the solution from the shelves.  While the fulcarium fungus did not appear to have invaded the containers once the solution was allowed to soak the hydrogel or silicone material it provided a culture medium that suited this generally available fungus.

There was no way this could have been predicted prior to
clinical usage and the mystery of the etiology of the infection eluded understanding for several months. The chemical used in the "ReNu" product is unique to B&L and no other lens solutions company has been noted.
